I believe legit, but a couple off things.

The script is half-right (upper case R, but script otherwise; seems odd). The bottle is the biggest problem. I've always seen it depicted as spherical with a line or two to designate fill line, but never with a darkened top/cork.

Even if he looks like a TV evangelist or an obscure cult leader, Vojo Šindolić had a great long conversation with Hank (a couple of them, actually) and published it in a book of Bukowski's selected prose called "Ispovijesti čovjeka" (Confessions of a Man) back in 1991.
I wish I had it in English to post it here, it's a great read.
